Summary
MAIN ROLE AND PURPOSE OF THE POST The Social Prescriber will work across Sparcells PCN practices. A Social Prescriber is a link worker who empowers people to take control of their health and wellbeing, through giving time, focussing on what matters to me and taking a holistic approach. RESPONSIBILITIES The Social Prescriber will: Receive and manage referrals from the clinical team and provide personalised support to those individuals. Work in partnership with the practice team, supporting patients and promoting positive lifestyle changes and preventing ill-health by means of behaviour change techniques. Complete holistic care planning assessments, analysing how needs can be met by services and other opportunities available in the community. Provide information and advice about a wide range of services to promote health and wellbeing and maintain independence within communities. Forge strong links with local VCSE organisations, community, and neighbourhood level groups. Contribute to reducing demand on statutory services, hospital admissions and combatting unnecessary practice appointments. Work with patients, in the practice or in their own homes, providing person centred care. Maintain detailed records of interactions within the patient medical record, ensuring that they are accurate and recorded contemporarily. Participate in multi-disciplinary meetings to plan patient care. Engage with patient suffering with Dementia, to offer support and enable them to attend annual review. Become part of the Brunel Social Prescriber network. Receive support and training from Community Health and Wellbeing & Live Well Swindon. TRAINING Complete the NHS England and NHS Improvement online learning programme, https://www.e-lfh.org.uk/programmes/social-prescribing/ Undertake a qualification recommended by the Personalise Care Institute, https://www.england.nhs.uk/personalisedcare/supporting-health-and-care-staff-to-deliver-personalised-care/personalised-care-institute/ Engage with peer support networks run by NHS England and NHS Improvements at local level. Participate in regular clinical supervision with a manager of the clinical team. Attend and participate in practice monthly clinical meetings. Engage in the practice appraisal process.
