Summary

Structured medication reviews: to be pre-booked either opportunistically by clinical team if recognised structured medication review is needed (and monitoring has already been arranged). Or by reception from lists provided e.g. starting with patients with more than 10 medications who have not had a review in the last 6 months, high risk groups such as patients on anticoagulants. Pharmacist can use admin time to pre-screen/ identify next group to be booked in with her. Further prescribing projects can be used to identify patients needing 30 minute or 20minute reviews with the practice pharmacist. Ardens structured medication review template to be used to code records Online consultations: queries about medications from patients via the website can be booked into these slots the pharmacist can either respond via text or telephone. MHRA + CAS alerts: pharmacist to use admin time to respond to alerts. Record to be updated via Teamnet. Medicines monitoring: Support admin/phlebotomy team in ensuring medication monitoring up to date. Reports can be viewed via Ardens searches on EMIS. Specific patients from these searches can be booked into SMR appointments where review of monitoring is needed. (Indication CQC targets and safety measures). When possible medication management on the day prescription requests can be viewed and any overdue monitoring/ reviews identified and actioned. AccuRx/Florey responses: These could be allocated to online consultation slots on the day by the tradmin doctor according to capacity. DOCMAN: letters have been screened by Admin and either sent to duty doctor to take action (where further management is needed or if solely medication reconciliation needed sent to pharmacist. Med reconciliation template to be used to record changes made to medication. If pharmacist is unclear of any of reconciliation needed to liaise with the tradmin doctor on the day.
