Summary

The Community Mental Health Nurse role is suitable for Registered Nurses with a Mental HealthNursing qualification who have successfully completed preceptorship and have registered as a practicesupervisor and/or assessor. You will need strong clinical skills in mental health assessment, safetyassessment and safety planning in-depth understanding of medication management and ability toprovide therapeutic interventions. You will be expected to operate within legislative frameworks Mental Health Act 2007, The Mental Capacity Act 2005, The Care Act 2014 and other relevant healthand social care legislation. The post holder will deliver or advise on outstanding nursing care that reflects the values of the Nursingand Midwifery Council (NMC) Code and AWPs commitment to trauma-informed practice. Thisincludes fostering a culture of kindness, respect, and inclusion in every interaction. Your role will involve leading on holistic assessments, collaboratively developing Personal Wellbeing Plans, and evaluating outcomes with service users, their families, and the wider Multi-Disciplinary Team. You act as a keyworker for patients, coordinating all aspects of care and communication for theperson, their family members and carers, and the services that are involved. You will maintain regularcontact with the person and their family members and carers and specify this in their PersonalWellbeing plan. You will support the physical and mental health of those in your care, uphold theirhuman rights, and contribute to a safe, therapeutic environment. You will also provide guidance and supervision to junior colleagues and students, modellingcompassionate leadership and reflective practice. You will will be expected to provide mentorship forstudent nurses, provide preceptorship and supervision for Band 5 nurses and supervise other juniorstaff as directed Accurate and sensitive documentation will be a key part of your role, ensuringcontinuity and quality of care.
