Job description
The interim 1 FTE ICM Live well post will focus on:
Carers support services review and extension
DP employment Support review and extension
Commissioning of extremely complex bespoke package of care for s117 eligible individual due for discharge in January 2027
Attendance at LDA DSR meetings and arranging care treatment reviews and attending NHSE LDA monthly meetings
Day opportunities review to inform future commissioning intentions including identification of savings
Sensory Support commissioning options paper
Supporting delivery of market management efficiencies in the ASC transformation programme
