Summary
To provide high quality nursing care and participate in the assessment, planning, implementation and evaluation of the care needs for the patient group.
To deliver compassionate person-centered care, and treat patients, service users, their families and their carers with consideration, dignity and the respect you would want for yourself or your family. The post holder will be expected to liaise with the linked inpatients wards (PAU/A21) within the acute setting and ED and UTC to identify complex patients for discharge and coordinate to facilitate safe and timely discharge is maintained. The post holder will be expected to deliver direct nursing care to patients in their own homes and within a clinical environment and deliver compassionate person-centered care, and treat patients, service users, their families and their carer’s with consideration, dignity and the respect you would want for yourself or your family.
Practice within theNMCStandards of Practice and behaviour for Nurses andMidwives. Participate in the assessmentofpatient careneeds,planning, implementing and evaluating programmesofcare in accordance with the agreed policies and procedures of The Royal Wolverhampton NHSTrust. Administer prescribed medication and treatment in a timely manner, in accordance with Trustpolicies and Professional guidelines. Take responsibility for groupsofpatients as required, with minimal supervision from seniorstaff. Deliver evidence-basedcare, and promote theuseofevidence-basedpractice, referring to Nursing Clinical Practices accordingly. Liaise effectively with the multi-disciplinary team to ensure timely deliveryofcare and develop good workingrelationships. Take personal responsibility for actions and omissions and recognise personalaccountability.
