Summary
Bridgewater Community Healthcare NHS Foundation Trust and Warrington and Halton Teaching Hospitals NHS Foundation Trust plan to integrate and form a new organisation on the 1st April 2026. As such, if your start date is on or after 1st April 2026, you will be employed by the newly formed organisation. Summary of Duties and Responsibilities Provide holistic care and health assessments for individuals, families, and the wider community. Support and coordinate complex or continuing care under direction and supervision. Maintain accurate clinical records and ensure effective communication with patients, families, carers, and multi-disciplinary teams. Demonstrate and maintain clinical skills, uphold professional standards, and participate in clinical supervision and audits. Promote equality, diversity, and evidence-based practice. Mentor nursing students and support training and development for team members. Fulfill administrative tasks including reporting absences, supplying information, and adhering to trust policies.
