Summary

Your role will involve assessing service user referrals, creating personalised care/support plans, and ensuring high-quality, tailored interventions. You'll focus on promoting independence, improving health and wellbeing, managing disease conditions, and reducing health inequalities, with an emphasis on palliative and end-of-life care when necessary. Using professional judgement, youll select appropriate care and treatment options, always considering individual circumstances and preferences. Youll lead by example, fostering skill-sharing and developing best practices within the team. Your compassionate approach will be essential, offering empathy and reassurance to service users and their carers, especially in challenging situations. You will also be tasked with working with those who find engagement difficult and managing uncooperative or challenging behaviour. Additionally, you will maintain accurate records, ensure safe equipment use, and lead the effective deployment of team resources. As a supervisor, you'll provide guidance, mentor junior staff and pre-registration students, and identify training needs for continuous professional development. You will also play a key role in performance and quality management, including data reporting, clinical audits, and the implementation of policies and procedures. Managing your own caseload and overseeing the workload of others will be a crucial part of your responsibilities, ensuring care delivery meets the Trusts high standards.
