Summary
Key Responsibilities Manage chronic disease care across QOF areas (cardiovascular, respiratory, diabetes), promoting self-management and making appropriate referrals. Deliver cervical screening in line with national guidelines. Provide childhood immunisations and the full vaccination programme under PGDs/protocols. Perform Doppler assessments (ABPI) to support vascular and wound care. Undertake a full range of treatment room duties including BP, BMI, urinalysis, peak flow, venepuncture, wound care, ECGs and other relevant procedures. Provide evidence based health promotion (lifestyle, diet, smoking cessation, women's and men's health, family planning). Maintain accurate electronic clinical records and contribute to audits and quality improvement work. Work collaboratively with nursing, clinical and administrative colleagues to deliver seamless patient care. Ensure treatment rooms are clean, well-organised and fully stocked. Provide cover for colleagues where required. Adhere to all infection control, health & safety and confidentiality policies. Undertake any other duties appropriate to the role.
