Summary

Duties of the Post As Consultant in Palliative Medicine you will provide the following: Deliver consultant level day-to-day medical care to hospice in-patients and patients accessing other Specialist Palliative Care services. Provide clinical leadership, education and development to the Advanced Clinical Practitioners Provide leadership and participate as a member at the weekly in-patient multidisciplinary team meeting. Responsible for review of current out of hours cover and implementation/ development of the robust out of hours cover. Cover for colleagues annual leave and other authorised absences. This post is alongside an in house GP working 2 days per week. Any responsibility which relates to a special interest. Educational and clinical supervision of trainee doctors, advanced clinical practitioners and supervision of Specialty Doctors. We currently do not have trainee doctors but this will be a future requirement. Training and supervision of Nurse Independent Prescribers. To participate in education and training junior doctors, medical students, hospice staff and volunteers. Participate in clinical audit, quality improvement projects and clinical governance processes. Support research activity. Comply with Continuing Professional Development (CPD) requirements as stipulated by the Royal Colleges. To keep a portfolio of evidence, to have an annual appraisal and write a personal development plan (PDP) in line with GMC revalidation policies and procedures. Support and assist in the achievement of Hospice strategy and objectives. Work with the leadership team, trustees and comissioners on the development of Palliative Care and End of Life Care service. Proposed Job Plan The initial job plan will be agreed flexibly with the successful candidate and will be subject to negotiation and review. Three months after commencement, the job plan will be reviewed and agreed between the appointee and the Director of Clinical Services. This will be a prospective agreement that sets out the consultants duties, responsibilities and objectives for the coming year, and will be reviewed annually. It should cover all aspects of a consultants professional practice including clinical work, teaching, research, education and managerial responsibilities. In addition, it should include personal objectives, including details of their link to wider service objectives, and details of the support required by the consultant to fulfil the job plan and the objectives. On call availability Out of hours activity is currently delivered by Derbyshire Health United (DHU) practitioners who then access on call consultants via South Yorkshire or Nottinghamshire dependant on area. This will be reviewed and developed in line with service need. It is possible that an on-call requirement may become necessary in the future. Supporting Programmed Activities The role will require oversight of clinical education for Palliative and End of Life Care across Bassetlaw Hospice and to colleagues in partner organisations, to ensure services are educated and fit for purpose. The appointee is expected to participate in teaching in collaboration with the education lead. The post holder will be encouraged to contribute to research activities at the hospice and contribute to audit and quality improvement projects. The appointee is expected to take part in continuing professional medical education; study leave is provided and funding is available on application. Appraisal Revalidation and Job Planning Consultants are expected to undertake the annual Job Planning process which is carried out in collaboration with the Director of Clinical Services. The Consultant will bring relevant data on workload in the previous year to the meeting. There is a new starter Trust induction programme for all staff and a New Consultant Induction Programme Prior to commencing post, the Consultant will be informed of the corporate and local induction processes and programme. It is expected that the Consultant will participate fully in this and keep up to date with mandatory and statutory training in line with Trust requirements. Newly appointed Consultants will be encouraged to seek a mentor to support their development in their first year of appointment.
