Summary

Main Duties & Responsibilities: Actively contributes and promotes a team approach to care, based on mutual support and respect. Actively listens and communicates with patients, families and carers via the telephone, virtually and face to face in a sensitive manner, recognising the need to pass on information to the Registered Nurses. Engage and positively deliver, with the assistance of volunteers, group activities of support and education aligned to the service in the teams weekly information and support groups. Provide cover for the Rugby Myton Support Hub as may be required. Actively engages in the use of Information technology (IT) to support patients and families where virtual support has been identified as the preferred method of contact. Ensures safe and responsible use of equipment; identifies and reports to senior colleagues any malfunctions and takes action to maintain safety as directed. Following appropriate training, have an awareness of available equipment that may need to be used within a group session, in accordance to Moving and Manual Handling Policy. Ensures that documentation is completed accurately and legibly in accordance with local guidance, and confidentiality is maintained using Caldicott Guidelines. To help make patients, relatives, carers and friends feel comfortable and welcome when attending the Outpatient setting at the Hospice site. Actively contribute to developing the service to enhance the care and support available to patients, carers and families. Be responsible for own continued development. Communication: Answer the telephone in a polite and courteous manner and with regards to confidentiality policy. Refer calls to colleagues as appropriate. Contribute to the support of volunteers ensuring effective communication is maintained. Demonstrate effective communication and work in partnership with patient, carers, Wellbeing Service nursing team and other members of the multi-disciplinary team, to ensure that care and support reflects individual needs. To alert the Wellbeing Service Lead/Senior Nurses of any concerns of crisis or problems arising with a patient or family/carer. Promote patients and carers as partners in care and treatment. Participate in the orientation of all new staff within the information and support group sessions. Under guidance from senior colleagues, develop excellent customer care skills, which will contribute to a positive profile of the Hospice. Contribute to the appropriate and efficient use of supplies and resources. Ensuring incidents and clinical adverse events and near misses are reported, and assist in the investigation at clinical level as required. To be conversant and comply with the policies and procedures of Myton Hospice including Confidentiality, Health and Safety, Fire and Child Protection. To promote a safe working environment under the Health and Safety at work Act (1974), ensure health and safety of staff, patients and visitors by complying with appropriate standards, including decontamination, infection control, COSHH, Medical Devices. Ensure the Day Unit environment is safe, clean and tidy, and adhere and comply with the Hospices Infection Control Prevention and Control Policy, Dress code and all other Hospice Policies and Guidance.
