Summary

1. Repeat Prescribing Management Process repeat prescription requests in line with practice protocols. Identify patients requiring medication reviews and arrange appropriate review with appropriate clinician. Resolve medication queries from patients, community pharmacies, and care homes. 2. Medicines Optimisation Undertake medication reconciliation following hospital discharge and outpatient appointments. Support with medication reviews within scope of competence/protocols. Identify and action prescribing safety alerts. Monitor high-risk medicines in accordance with local and national guidance. Support implementation of local formularies and prescribing guidelines. 3. Patient Safety & Clinical Governance Support systems for monitoring and recall of long-term condition medications. Assist with audits related to prescribing and medicines safety. Contribute to significant event reviews involving medicines. Ensure compliance with CQC standards relating to medicines management. 4. Patient Support Provide advice to patients regarding medication use, synchronisation, and adherence (within competency). Support patients with medication queries and signpost appropriately. Promote self-care and understanding of prescribed treatments. 5. Care Homes & Vulnerable Patients Support medication reviews and prescription processes for care home residents. Liaise with care home staff and community pharmacies to resolve medication issues. 6. Collaborative Working Work closely with GPs, nurses, clinical pharmacists, practice dispensary and administrative staff. Liaise with community pharmacies and secondary care colleagues. Participate in multidisciplinary team meetings where appropriate. 7. Information Management Maintain accurate and contemporaneous patient records. Use clinical systems (e.g. EMIS, SystmOne) effectively. Ensure data protection and confidentiality in line with GDPR requirements. 8. Professional Development Maintain registration with the General Pharmaceutical Council (GPhC). Participate in appraisal and continuing professional development (CPD). Work within competence and escalate appropriately.
