Summary
Principle Responsibilities: Be able to demonstrate not only practical knowledge but also have a critical understanding of the range of theories and principles that underpin general practice. Asses patients presenting with undifferentiated, undiagnosed presentations and use advanced assessment, diagnostic reasoning skills and a range of diagnostic support tools to manage, treat or refer these patients appropriately. Support learning in practice settings and collaborate with education service providers and commissioners to ensure workforce and student needs are met where required. Be able to display originality and utilise this in innovative service development and delivery. Safely implement new policies and guidelines within the practice. Clinical Responsibilities: Assess, diagnose, plan, implement and evaluate treatment and interventions and care for patients presenting with undifferentiated diagnosis, complex needs, long-term conditions, acute and chronic illnesses. Maintain accurate and up to date clinical records. Contribute to clinical audits and quality improvement initiatives. Practice in accordance with their respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions and omissions at this level of practice. Independently prescribe and review medication for therapeutic effectiveness, appropriate needs and in accordance with current guidelines. Provide information and advice on prescribed or over the counter medication, medication regimens, side-effects and interactions. Proactively identify, diagnose and manage treatment plans for patients at risk of developing along-term conditions. Demonstrate a critical understanding of their broadened level of responsibility and autonomy and limits of competence and professional scope of practice. Act on professional judgement about when to seek help from GP partners. Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care. Work in partnership with individuals, families and carers. Using a range of assessment methods as appropriate. Work collaboratively with an appropriate range of multi-agency and inter-professional resources. Deliver co-ordinated care within the community and reduce the need for hospital admission. Act as a clinical role model for developing and delivering care that is responsive to the changing requirements. Informed by the local population, health needs agencies and networks. Learning and Management Be a role model for the practice, demonstrating a person-centred approach to service delivery and development. Evaluate own practice and participate in multidisciplinary service and team evaluation demonstrating the impact of advanced clinical practice on service function and effectiveness. Lead new practice design and evaluate the progress, changing as necessary. Demonstrate team leadership, resilience in managing situations that are unfamiliar, complex or unpredictable and seeking to build confidence in others.
