Summary
The childrens community team cover a seven day service from 8am-9pm.We support nursing care in the community to facilitate early discharges, prevent hospital admissions or readmissions and as support children with health needs in their school. We support education staff to meet the health needs of children by training them and developing care plans and work alongside other multi agency teams to support children to achieve best outcomes. This post is for 28.5 hours per week on a fixed term 12 month basis. Daily access to your own vehicle for managing community visits is required.
