Summary

Communication To work closely and collaboratively with other members of the practice management team to achieve high quality, safe and evidence-based patient care consistently across all sites of Tower Family Healthcare To work collaboratively with both clinical and non-clinical staff to deliver seamless care to the patients of Tower Family Healthcare. To work collaboratively with the ICBs Medicines Optimisation Team to support the implementation of medicine related ICB initiatives within the practice. To liaise closely with local community pharmacists, community nurses, care home staff, hospital pharmacists and other care providers, as necessary to ensure accurate exchange of information/clarification of queries regarding medicines. To communicate effectively to a variety of healthcare professional, patient, and community groups. This may involve small group sessions, public speaking or making presentations as appropriate. To ensure good quality data by recording, promptly and accurately clinical, and non-clinical information within agreed timescales to the health record or the appropriate clinical or non-clinical information system. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately. In the performance of the duties outlined in this job description, the post-holder will have access to confidential information relating to patients and their carers, member practices, staff and other healthcare workers. All such information from any source is to be regarded as strictly confidential. Information relating to patients, carers, colleagues, other healthcare workers or the business of the organisation may only be divulged to authorised persons in accordance with the policies and procedures relating to confidentiality and the protection of personal and sensitive data. Keep up to date with IG And GDPR legislation Delivering a quality service Recognise and work within own competence and professional code of conduct. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures. Prioritise, organise and manage own workload in a manner that maintains and promotes quality and meets practice expectations. Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation. Participate in the maintenance of quality governance systems and processes across the organisation and its activities. In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate. Evaluate patients response to health care provision and the effectiveness of care. Support and participate in shared learning across the practice and wider organisation. Actively participate in the practice significant event procedures. Ensure learning and actions from significant event reviews are implemented and reviewed. Understand and apply legal issues that support the identification of vulnerable and abused children and adults and be aware of statutory child/vulnerable patients safeguarding health procedures and local guidance. Maintain high standards of infection control at all times, protecting patients, self and colleagues. Personal and people development Take responsibility for own learning and performance including participating in clinical supervision and acting as a positive role model. Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice Critically evaluate and review innovations and developments that are relevant to the area of work. Prioritise own workload and ensure effective time-management strategies are embedded within the culture of the team. Work effectively with others to clearly define values, direction and policies impacting upon care delivery. Discuss, highlight and work with the team to create opportunities to improve patient care. Managing information Use technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information. Understand responsibility of self and others to the practice regarding the Data Protection and Freedom of Information Act. Health, Safety & Security The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the Practice Health & Safety Policy, the Infection Control policy and published procedures to include: Using personal security systems within the workplace according to Practice guidelines Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks Making effective use of training to update knowledge and skills Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards Actively reporting of health and safety hazards and infection hazards immediately when recognised. Keeping own work areas and general / patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holders role. Undertaking periodic infection control training. Reporting potential risks identified Conform to health and safety legislation, taking the necessary actions to manage risks and acting as a role model in promoting health, safety and security Attend statutory and mandatory training at the required times to maintain and update skills.
