Summary

This is a generic Job Description for Advanced Clinical Practitioners (ACPs) workingwithin the Trust. Specific responsibilities will vary between specialties, however allACP posts will be expected to reflect the four pillars of advanced practice: ClinicalPractice, Leadership, Education of self and others, and Research & Development.a)Work to a high level of personal and professional autonomy, using specialistknowledge and skills to deliver independent assessment, clinical examination,diagnosis, clinical decision making and planning of care and treatment, healthpromotion and safe admission and discharge of patients. This may include:b)on-call/out-of-hours and in-hours/bleep-holding responsibilities, as part of the agreed job plan.Act as an independent non-medical prescriber, where applicable to role/professionWork collaboratively with all members of the multidisciplinary team to providebest clinical care, emotional and psychological support and education to patients andtheir carers.c)Facilitate evidence based practice, and promote and undertake research andclinical audit within the service and across the Trust. Design and provideclinical/practical and theory-based education and training, eg. to nurses/AHPs, juniormedical staff.d)Work with innovation and flexibility to adapt to a fast-changing specialty, andplay an integral role in the operational/strategic development of the service. The Advanced Clinical Practitioner (ACP) will provide expert assessment, treatmentand management of a case load, using complex communication skills, judgement,discretion and decision making. They will monitor and assist with improvingstandards of care, supervising, teaching and supporting colleagues providing skilledprofessional leadership. Where appropriate they will act as an expert in their field andlead and/or participate in initiatives to improve standards of care for the client groupand assisting to advancing the Frailty ACP role.The Advanced Clinical Practitioner is service driven, and the role is structured aroundthe Department of Health Guidance across the four domains of Advanced Practice,those being: 1.
