Summary

Management of the Prescribing team and system Manage the prescribing team, including prescribe clerk and other admin staff involvement in the preparation of prescriptions Manage the system for repeat and acute prescription requests, including optimising efficiency and safety Clinical Pharmacist-Led Clinics Undertake structured medication reviews (SMRs) and clinical medication reviews Conduct post-discharge reviews and medicines reconciliation for patients returning from secondary care, ensuring continuity and safety. Support GP and nurse led clinics for long-term condition (LTC) management, including but not limited to hypertension, diabetes, COPD, asthma, atrial fibrillation, heart failure, and chronic kidney disease. Medicines Optimisation Lead on medicines optimisation across the practice, ensuring patients receive the best possible outcomes from their medicines. Review and implement national and local prescribing guidance, engaging with UHB requirements for QI and (Medicines Management Incentive Scheme (MMIS) Identify and manage high-risk medicines (e.g., lithium, methotrexate, warfarin, DMARDs) and ensure appropriate monitoring and recall(s) are in place. Support the management of drug safety alerts, MHRA communications, and medicine recalls in a timely and systematic manner. Lead on antimicrobial stewardship initiatives, reviewing antibiotic prescribing data and implementing improvement strategies. Collaborate with community pharmacies, secondary care, and UHB colleagues to improve medicines pathways and reduce waste. Consultation and Clinical Assessment Conduct comprehensive clinical assessments using history-taking, clinical examination (where trained), and interpretation of investigations. Use clinical decision-support tools and point-of-care testing where appropriate. Provide expert advice and second opinions to other clinicians regarding medicines, drug interactions, and therapeutic decisions. Manage enquiries from patients, carers, and clinical colleagues relating to medicines, safety concerns, and therapeutic options. Quality Improvement and Clinical Governance Lead pharmacy-related quality improvement projects and clinical audits, presenting findings and recommendations to the wider practice team. Contribute to the practice's clinical governance framework, significant event reviews, and repeat prescribing policies. Lead on/participate in QI projects both national and local Monitor and report prescribing data using audit+/EMIS reporting and other tools to identify outliers and opportunities for improvement. Develop and maintain practice-level prescribing policies, protocols, and standard operating procedures (SOPs). Actively participate in HIW inspections and other quality and regulatory processes. Digital and Data Use clinical IT systems, including EMIS Web, proficiently to manage patient records, produce prescriptions, and document consultations. Utilise population health management tools to identify at-risk cohorts and support proactive care planning. Support digital transformation initiatives relating to electronic prescribing, repeat prescribing, and online consultation services. Independent Prescribing Practise as an independent prescriber within scope of competence and relevant clinical area(s), in accordance with GPhC standards and the practice prescribing policy. Conduct face-to-face and remote consultations for acute and long-term conditions, issuing prescriptions independently where appropriate. Manage and review prescriptions for complex patients with multi-morbidities, including the initiation, continuation, and cessation of medicines. Participate in prescribing audits and peer review processes to assure the quality and safety of independent prescribing practice. Manage practice interactions with local community pharmacies Establish and maintain good working relationships with our local community pharmacies Proactively address issues around the ordering of repeat medications to ensure systems are safe and support patients access to essential medication Create a system to minimise the impact of unavailable medications
