We are offering 6-8 sessions for a Partner or a Salaried GP with a view to Partnership. Nestled between the sandy surfing beaches of Woolacombe and Croyde, and the rugged cliffs of Exmoor, we are perfectly situated for those looking for an amazing outdoors lifestyle whilst also delivering high quality patient care. Combe Coastal Practice is one the largest Practices in North Devon with a list size of 18,200. Our clinical team comprises Partner and Salaried GPs, a large nursing & patient coordinator team, and an ever-expanding team of multidisciplinary professionals viathe PCN ARRS scheme including Paramedics, Clinical Pharmacists, FCPs, Mental Health Nurses and Health & Wellbeing Team. Patient care is delivered from our main surgery in the harbour town of Ilfracombe and 2 branch surgeries in the coastal villages of Woolacombe and Combe Martin. Key strengths of our practice include: Close working relationships within our North Devon Coastal PCN Individual partner patient lists to facilitate good continuity of care. High patient satisfaction scores Dedicated urgent care, chronic disease management, womens health, minor surgery, pharmacy, first contact physio, mental health and health & wellbeing teams. Outreach clinic for the homeless/vulnerably housed in Ilfracombe. Job Description 1) Clinical responsibilities Provide comprehensive primary medical care to registered patients, including assessment, diagnosis, treatment, prescribing and referral as appropriate. Undertake a mix of face-to-face, telephone and online consultations in line with the agreed job plan. Provide home visits when clinically appropriate and in line with our duty/visiting model. Manage acute presentations and contribute to same-day demand management according to practice and Urgent Care Team Provide ongoing care for patients with long-term conditions, including medication reviews and evidence-based management. Maintain accurate, contemporaneous clinical records and correspondence in line with NHS and GMC requirements. 2) Administrative and professional responsibilities Complete clinical administration arising from consultations in a timely manner (e.g. results, referrals, letters, tasks), in line with the agreed job plan. Participate in practice meetings, clinical meetings and multidisciplinary discussions as required. Contribute to the practices clinical governance activity, including audits, significant event reviews and learning from incidents/complaints. Support the training and development of the wider team where appropriate (e.g. medical students, GP trainees, ARRS staff), subject to experience and job plan. 3) Quality, safety and governance Work within practice policies and procedures, including safeguarding, infection prevention and control, information governance and prescribing policies. Contribute to maintaining standards required by the Care Quality Commission (CQC). 4) Teamworking and communication Work collaboratively with GP colleagues, nursing team, practice management and the wider PCN multidisciplinary team. Communicate effectively with patients, carers and colleagues, including shared decision-making and safety-netting. Support a culture of mutual respect, continuous improvement and safe workload management. 5) Continuing Professional Development (CPD) and appraisal Maintain GMC registration, revalidation and annual appraisal requirements. .
