Summary
Care coordination under the Mental Health Measures Wales Working to the Mental Health Act 2005, this will be updated in February 2026. Planning care, goal setting, risk management and risk formation. For a holistic care and risk management Communicating with patients, carers, family including muti professionals e.g. Children services, MARAC, police, GP's, third sector, LMPHSS, HTT, MASH. Documentation/ongoing referrals are essential and there will be many CMHT, MHA etc documentation/referrals that will/maybe needed to be completed e.g. Tribunal reports, CTP, WARRN, assessment tools, daily notes and ongoing referrals. The role does require lone working and the CMHT ensure that staff's safety in paramount and therefore have measures in place to help reduce any risk to staff.
