Summary
You’ll work as part of a friendly multi‑disciplinary team who deliver high‑quality cardiac and pulmonary rehabilitation. Your work will help people understand their condition, recover well after a cardiac event and feel more confident in managing their health at home.
What you’ll do:
• Assess people’s rehabilitation needs and plan safe, tailored nurse‑led care
• Support patients, families and carers with clear information that aids self‑management
• Work with colleagues across specialist nursing, community teams and secondary care to coordinate care
• Monitor progress, review risks and update care plans to keep people safe
• Keep accurate records and contribute to service improvement and audit
What we’re looking for:We welcome newly qualified and experienced nurses who want to develop in cardiac care. You’ll need to be a Registered Adult Nurse with knowledge of cardiology services and evidence‑based practice. You should be confident communicating sensitive information, able to work both independently and within a team, and committed to ongoing learning. Basic IT skills and the ability to travel across the service area are essential.
