Summary
Job description Responsible for coding of relevant information to the patients electronic healthcare records. Read all incoming correspondence to the practice, accurately coding the relevant diagnosis, procedure, information into the patients records. Ensure that follow-up actions, tests are brought to the attention of the relevant staff. Scan patient-related documents onto their medical records using codes as agreed by the clinical team. Ensure incoming letters are scanned onto the patient record. Ensure all QOF related data is captured. Ensure that all clinical codes are current. Ensure that patient medical problems are grouped and combined appropriate so that the medical records are always tidy. Follow practice protocols to ensure consistent and high-quality care is provided to our patients. Correct errors in filing and ensure corresponding records are amended accurately. This job description is not meant to be exhaustive. It describes the main duties and responsibilities of the post. The candidate would be expected to undertake any other duties commensurate with the role as agreed with the practice. On occasion you will be required to help with reception duties this will include, answering the telephone and always dealing with appointment requests and any other patient query in a courteous manner. You may be required to work in the reception office dealing with patients and other visitors to the surgery. To participate in the administrative responsibilities of the practice team: Ensure accurate records of all consultations and treatments are recorded in the patients computer records. Ensure accurate completion of all necessary documentation associated with patients health care and registration within the practice. Ensure the collection and maintenance of accurate statistical information required for regular and ad-hoc reports. Attend and participate in meetings as required. Assisting in the formulating and strategy of practice policy. Record accurately and adhere to the system for repeat prescription policy and refer where appropriate to the GP. Edit and summarise notes when prompted to do so and use recognised codes to record information.
