Summary
Take routine bloods, ECG, Complete Clozapine Assay, physical health monitoring i.e. BP, pulse, weight height. However, the post holder is not expected to interpret results of bloods tests, ECGs Offer health promotion, dietary advice, healthy eating, smoking cessation advice, substance misuse referring to appropriate agencies in agreement with the service user. Ensure appointments are re-booked within NICE guidelines and in a timely manner according to clinical need. Communicate outcome of physical health screening to GPs & PCNs highlighting any physical health concerns Maintain effective management of clinical areas including, co-ordinate ordering to ensure adequate stocks, safely dispose of sharp bins, equipment check i.e. defibrillator, and calibration of specific physical health equipment. Signpost to appropriate alterative services such as PCMHT and VCSE pathways. Develop a rapport with service users within appropriate boundaries. To encourage service users to be fully involved and engaged with their agreed care plan and to access appropriate services. Implement agreed programmes of care collaboratively with service users and carers under the supervision of a named professional. Help to identify early signs of relapse by monitoring the service user's progress, level of functioning and mental state and alert the appropriate staff involved in their care. To take a role in evaluation of care provided To identify and support carer needs as required. To take a role in health promotion including physical health needs. Participate in and help run social activity groups.
