Summary
The postholder will work as part of a primary care based neurodevelopmental service, providing support to adults seeking advice or assessment for ADHD. They will act as the first point of contact within primary care, offering guidance to individuals experiencing challenging symptoms and helping them understand available assessment pathways. A key element of the role is the completion of needs led stratification assessments to identify presenting issues and support patients in accessing appropriate education, advice and intervention options. Under appropriate supervision, the postholder will also undertake annual ADHD reviews, including physical health checks and discussions around medication related concerns. All clinical and appointment related administration must be completed accurately and in line with service standards. The postholder will receive, screen, triage and stratify referrals for adult ADHD assessments or ongoing treatment, conducting both face to face and occasional remote appointments. They will carry out comprehensive assessments in accordance with relevant guidance, best practice and the service model, identifying needs arising from ADHD or other contributing factors. They will provide evidence based education, advice and guidance to help patients understand and manage symptoms, while managing a defined caseload in collaboration with other professionals, agencies and carers to support engagement with appropriate interventions. Working under supervision, the postholder will complete annual ADHD reviews, including physical examinations, discussions around symptom management, side effects and nonpharmacological strategies, and will liaise with GPs and other agencies as required. A holistic and empathetic approach is essential, ensuring that both physical and mental health needs are considered and that patients are signposted or referred to other services when appropriate. Clear, accurate and timely clinical records must be maintained, summarising appointment outcomes and recommendations for ongoing support or referral. The postholder will develop a strong understanding of the patient journey from referral through assessment, treatment options and shared care arrangements. They will support effective collaborative working across general practice, the primary care service and secondary care through clear communication with colleagues. The role also requires engagement with web based resources relevant to this patient group, ensuring information remains accurate and up to date for both colleagues and patients. The postholder will remain informed about current research and developments in ADHD assessment, treatment and mental health, and will work autonomously within the remit agreed in clinical supervision and the service model. They will also plan and coordinate necessary investigations, including physical healthcare needs for service users in the community. In addition to core duties, the postholder will ensure their actions contribute to maintaining a high quality service. They will take responsibility for their own professional development, participating in training and maintaining up to date knowledge relevant to the role. They will engage in annual performance and development reviews and undertake any identified training, including all statutory and mandatory requirements set by the Practice or Primary Care Network. Adherence to all primary care and PCN policies and procedures is essential. The successful candidate will work across all practices within the PCN.
