Summary
1. ADHD Reviews & Monitoring Deliver annual physical health monitoring (BP, pulse, weight/BMI). act as first point of contact and initial assessment for patients seeking a diagnosis of adult ADHD Complete annual ADHD holistic reviews in line with NICE NG87. Monitor symptom control, medication adherence, and side effects. Escalate clinical concerns to specialist ADHD GP. Maintain accurate EMIS records and apply required SNOMED coding. 2. Psychoeducation & Self-Management Provide structured psychoeducation and coaching in: -Executive functioning and organisation -Time management strategies -Sleep hygiene -Emotional regulation -Workplace and educational adjustments Support access to digital self-management platforms. Signpost to Talking Therapies, social prescribing teams, and voluntary sector support. 3. Risk & Safeguarding Undertake structured mental health and risk assessments. Identify substance misuse or safeguarding concerns. Escalate appropriately in line with PCN safeguarding policies. Recognise when referral to secondary mental health services is required. 4. MDT & Integrated Working Participate in multidisciplinary discussions across the PCN. Liaise with secondary care ADHD providers for advice and guidance. Contribute to KPI reporting and service evaluation.
