Summary

The Personal Adviser will provide high-quality information, advice and guidance tailored to the individual needs of the young person, supporting them to develop the skills and confidence needed for independent adulthood. They will build positive and supportive relationships to help young people identify and achieve their goals.

The Personal Adviser will complete assessments of need to inform each young person’s Pathway Plan and take a lead role in its development, coordination and review. They will monitor progress against the plan, ensuring support remains relevant and responsive to the young person’s changing needs.

This role involves working in partnership with a range of internal and external agencies, including other local authorities, to ensure effective implementation of the Pathway Plan. The Personal Adviser will also advocate on behalf of young people, ensuring their views, wishes and feelings are central to planning and decision-making.

This post is a one one year fixed-term contract.

Candidate Profile

The candidate will have experience of working with young people with care experience.
