Summary
Support patients to get the best from their medicines, including switching medications in line with agreed protocols. Improve repeat prescribing processes in general practice, including promotion of repeat dispensing and online ordering. Minimise clinical risk and work to reduce wasted medicines. Work as part of a multidisciplinary team in a patient-facing role, clinically assessing and treating patients using in-depth clinical knowledge. Be directly responsible for the care and prescribing of defined cohorts of patients. Deliver and develop pharmacy-led medicines optimisation services within general practice and across the wider multidisciplinary team. Ensure practices remain up to date with medicines management guidance and best practice. Lead and promote medicines optimisation within specialist areas, ensuring best value, reduced harm, and improved patient outcomes. Lead and deliver evaluation and audit of service delivery to maintain and improve service quality. Work in line with Medicines Optimisation principles, national guidelines, and the latest evidence base. Preference will be given to candidates who are CPPE pathway trained, or candidates must be willing to undertake CPPE training and enrol within six months of employment if not already completed.
