Job Overview
Discovery Homecare is seeking an organised, compassionate, and proactive Care Coordinator to join our growing team. The Care Coordinator plays a key role in ensuring that clients receive high-quality, person-centred care by effectively coordinating care staff, schedules, and services. The role involves working closely with care workers, clients, and management to maintain excellent standards of care and operational efficiency.
Key Responsibilities
• Coordinate and schedule care workers to ensure all client visits are covered effectively.
• Maintain and update care schedules, rotas, and client records.
• Liaise with clients and their families to ensure care needs are understood and met.
• Support the recruitment, onboarding, and supervision of care staff where required.
• Monitor service delivery to ensure compliance with company policies and regulatory standards.
• Respond promptly to changes in care needs, emergencies, and staff availability.
• Conduct regular reviews of care packages and communicate updates to relevant stakeholders.
• Ensure accurate documentation and reporting within internal systems.
• Work closely with management to improve service quality and client satisfaction.
• Provide support and guidance to care staff to ensure best practices are followed.
• Undertake on-call responsibilities as part of the rota.
Requirements
• Previous experience in a care coordination, scheduling, or healthcare support role (preferred).
• Good understanding of domiciliary/home care services.
• Excellent organisational and time management skills.
• Strong communication and interpersonal abilities.
• Ability to work in a fast-paced environment and respond to changing priorities.
• Competent in using scheduling systems and general IT software.
• A compassionate and professional approach when working with clients and staff.
Desirable
• NVQ/QCF Level 3 in Health & Social Care (or working towards).
• Experience working in a homecare or community care environment.
• Knowledge of care compliance and regulatory requirements.
