The post holder provides high-quality, patient-centred mental health care, acting as a keyworker for a group of patients. Responsibilities include assessment, planning, implementation, and evaluation of care using the Personalised Care Framework (PCF). The role involves regular leadership duties, including the Nurse In Charge role and mentoring/supervision of junior staff. Key Responsibilities Clinical Deliver individualised care plans in partnership with patients, carers, and multi-agency professionals. Conduct risk assessments and maintain accurate electronic records. Administer medication safely and educate patients/carers on its use. Work within NMC Code, Mental Health Act, and Livewell Southwest policies. Support and involve carers in care planning. Managerial Act as Nurse In Charge of the shift, planning the daily activity which may include covering sickness and adjustment of rota. Supervise and appraise junior staff; promote staff development. Ensure compliance with clinical and non-clinical policies (e.g., Health & Safety, COSHH). Manage resources, petty cash, and stock control. Professional Development Undertake mandatory training and maintain professional competence. Participate in practise supervision and appraisal. Stay updated with evidence-based practice and research. Governance & Compliance Contribute to audits and clinical governance. Adhere to infection control, safeguarding, and risk management policies. Support sustainability initiatives and comply with no-smoking/alcohol policies.
