An opportunity has arisen for a practice manager to join the Health and Social Care Connect (HSCC) team based in Eastbourne. As the service responsible for providing the point of access into adult community health and social care services in East Sussex, we are looking for an experienced manager with an interest in ongoing service development and delivery.

HSCC provides a variety of functions including processing professional health referrals, providing the public with information and advice, completing social care assessments and the triage of safeguarding concerns and enquiries. We work closely with partners across the health and social care system and the emergency services, and as a practice manager you may be required to represent HSCC at various meetings and forums.

HSCC is an office based service that uses IT and telephony systems to deliver the right outcome, at the right time for the residents of East Sussex. We are looking for an experienced social worker to join the team to support staff and service delivery.

You will also have line management responsibility for staff who provide a 24/7 service. Whilst you will not be required to work shifts you will be required to participate in the on-call rota out of hours to provide management support and advice.

HSCC continues to develop and evolve and you could play a key role in this, helping us to build on the successes of the service to date.
