Summary


To provide professional leadership and support to the Enhanced care/prevention of Delirium (POD) team based within the acute Trust. By working collaboratively with clinical areas utilising the available resources to provide an enhanced level of patient centred care. To support the dementia and delirium training, bringing a skilled workforce in dementia and delirium to the organisation.

The post holder provides senior clinical, professional and operational leadership for dementia and delirium services, focusing on enhanced care, delirium prevention and engagement initiatives across the Trust. The role leads the Enhanced care team (ECT) to deliver high-quality, person-centred and dignified care for people living with dementia or at risk of delirium.

The post holder leads the A-level and further education student enrichment programme, working with local colleges to recruit, induct and train students, and supports the development of the volunteer workforce across clinical areas. Advanced clinical expertise are required to assess patients with, or at risk of, delirium and requiring enhanced therapeutic observation, while supporting ward teams to develop individualised care plans and ensuring effective deployment of the ECT.

Working in line with national and local guidance, the post holder acts as a clinical role model, leading Trust-wide education in dementia, delirium prevention, the Butterfly Scheme and John’s Campaign. The role involves close collaboration with multidisciplinary teams, community services and third-sector partners to improve continuity of care.

The post holder also holds managerial responsibility for recruitment, performance reviews, rota planning, workforce management, risk and complaint handling, service evaluation and development, ensuring care is evidence-based and delivered in line with the NMC Code.

• Lead and support the Enhanced and Engagement Care Support Worker

(ECSW)/Prevention of Delirium (POD), team to provide professional nursing leadership within the team.



• To manage the A level student enrichment programme, engaging with the enrichment leads in local colleges, promoting and supporting spread across the localities. To develop a wider student enrichment working with local colleges.



• To lead the recruitment, induction and training for A level students and students in further education



• Expand the volunteer base to work within the hospital setting and spread the prevention of delirium to all clinical areas.



• To undertake the clinical assessment of people with/ or at risk of delirium. To advise on the patient centred care planning process, supporting teams to provide individualised and dignified care.





• To undertake a clinical assessment of people requiring enhanced therapeutic observations and care, ensuring their individualised care needs are met.



• To complete daily allocations of enhanced care team, to provide enhanced therapeutic observations and care alongside the ward team.





• To demonstrate up to date knowledge of and work within national and local dementia guidance.



• To work in partnership with other professionals to influence change and innovation in dementia and delirium care.



• To work closely with the matron for dementia and delirium to ensure ongoing development of skills and knowledge through relevant training, conferences and collaborative meetings.



• To be actively involved in the delivery of training Trust wide, in line with national guidance for dementia, delirium, the Butterfly scheme and Johns Campaign.



• Work in collaboration with third sector organisations from the voluntary sector and community settings.



• To work towards patients living well at home with or at risk with delirium. To work closer with community services working collaboratively to deliver this through training and education.



• To produce reports on service provision and evaluation of the service delivery in line with patient/carers experience and outcomes.





• As required build a business case for future service provision or expansion.



• Participate in multi-disciplinary discussions, contributing with the decision-making process as advocate for the person.


Clinical Duties


• Present as a dynamic, innovative, and enthusiastic role model providing leadership, guidance and advice on both operational and professional issues.



• Actively participate in social engagement activities with patients and carers providing leadership for colleagues and student volunteers.





• Welcome carers to stay with patients living with dementia, supporting Johns Campaign throughout the Trust.

• Contribute proactively to service development ensuring the best use of all resources using effective benchmarking systems, ensuring all development enhance the patient experience.



• Work in partnership with ward teams, college enrichment leads and community teams to ensure the delivery of quality care.



• To be able to select, develop and motivate a team.



• To lead a team towards local, Trust and national objectives.



• To recognise, prevent and defuse potential areas of conflict and take a key role in the management of the outcomes.



• To be able to prepare and present reasoned discussion based on facts, convince others and resolve claims for competing resources.





• To work within the NMC code of professional practice and within policies, procedures and guidelines of Calderdale and Huddersfield NHS Trust.



• To act as a clinical role model, ensuring clinical expertise and practice is enhanced.


Professional Duties


• To establish and maintain effective communication within the multidisciplinary team, including contributing to effective communication concerning the specialism.

• To delegate work to match capabilities and workload of team members.

• To ensure that high standards of nursing care are given and maintained, and act when standards are not being maintained.

• To act as an innovative and enthusiastic role model, providing leadership, guidance and advice to staff on operational and professional issues. To promote an open, honest and transparent culture.

• To participate in the assessing, planning, implementation and evaluation of individualised programmes of care.


Managerial Duties


• To ensure an appropriate skill mix when planning rotas, delegating tasks, etc.

• To manage human and other resources effectively within budget allocation.

• To supervise and deploy staff where and when necessary to meet the continuing needs of the services.

• To undertake PDRs for team members.

• To lead on the recruitment and selection of staff in line with Trust policy, within own area.

• To attend and participate in meetings as required, including providing feedback and disseminating information as appropriate.

• To utilise information systems to plan and forecast future needs and ensure appropriate action.

• To monitor sickness, absence and competence issues and implement Trust policies, e.g. managing ill health, capability

• To ensure systems are in place for risk assessment and minimisation, including promoting a “no blame culture” in order that incidents and near misses are reported.

• To manage complaints within the team and contribute towards responses for complaints /concerns raised involving care delivered by the team.

• To participate in the investigation of incidents/complaints as required, including preparing detailed reports/letters to complainants.

• To participate in the supervision and teaching of pre/post registration students.

• To ensure systems are in place to support staff in development, e.g. mentorship, preceptorship.

• To identify training needs for team.

• To support the development of the ward/department team.

• To ensure the clinical learning environment is conducive to effective learning.

• To create and maintain networks beyond the ward/department, sharing good practice and information.

• To promote and lead Patient and Public Involvement strategies in the clinical area.

• To ensure patient information related to specialty is accurate, up-to-date and accessible for patients.

• To participate in research and the development of initiatives in practice and encourage others.

• To ensure clinical practice is evidence-based and act when standards are not being met.

• To identify the need for, participate in and support others to participate in, audit, ensuring agreed results are implemented.

• To lead in the standard-setting process, maintain consistency and take appropriate action where necessary.












Financial Duties


•To be proactive in the development of practice/services in the clinical area, through business planning process/service development, in conjunction with General Manager and Matron.


Managing Self


• Participate in regular supervision.

• Attend all mandatory training.

• Participate annually identifying, developing and agreeing your own development plan with your Line Manager using the Trust Appraisal.

• Comply with all Trust policies, procedures and protocols.

• Carry out duties with due regard to the Trust’s Equal Opportunity Policy.

• Seek advice and support from Line Manager whenever necessary.

• Maintain professional conduct including appearance at all times.

• Ensure maintenance of Professional Registration.
