Summary
Care Navigator Key Duties and Responsibilities Patient Support and Navigation Welcome and assist patients in a professional and friendly manner. Signpost and guide patients to appropriate services within the practice, Primary Care Network, and local community. Book and manage appointments across clinical systems; handle online requests and telephone enquiries. Process referrals, screening invitations, and community service bookings. Reception and Administrative Duties Front desk duties including meet and greet, handling telephones, emails, and patient enquiries. Register new patients and support access to online services. Maintain reception and waiting areas, manage deliveries, and ensure supplies are stocked. Assist with clinic preparation, including room setup and organisation of clinical equipment. Process and maintain accurate patient records, medical correspondence, and clinical data in electronic systems. Prescription and Clinical Support Support prescription requests and medication administration when needed. Assist with pathology results, clinical messages, and other administrative tasks for clinical staff. Health, Safety, and Compliance Maintain confidentiality and data protection standards in all tasks. Follow infection control procedures and manage specimens and clinical waste safely. Report incidents, support complaint investigations, and comply with health and safety regulations. Teamwork and Professionalism Work collaboratively with reception, administrative, and clinical teams. Provide excellent customer service and support practice operations efficiently. Participate in ongoing training, including chaperone and first aid if required. Please see the attached Job Description for the full duties.
