Description
The Social Work Coordinator (SO1) plays a crucial role in connecting individuals and families with necessary resources and support services in our community. This position involves coordinating social work activities, facilitating programs, and advocating for clients' needs. The ideal candidate will work collaboratively with healthcare providers, government agencies, and community organizations to ensure comprehensive service delivery and improve client outcomes. Responsibilities include assessing client needs, developing individualized service plans, and connecting clients with appropriate resources such as housing, employment, mental health services, and educational opportunities. The Social Work Coordinator will also be responsible for tracking progress and outcomes, providing ongoing support, and adapting plans as necessary to meet changing client needs. A strong focus on client advocacy and empowerment is essential, as well as a commitment to ethical practice and confidentiality. Additionally, the coordinator will engage in community outreach efforts to raise awareness of available services and promote social justice initiatives. Responsibilities
Coordinate social work activities and ensure effective service delivery.
Conduct assessments to identify client needs and develop individualized service plans.

Monitor and evaluate client progress, making adjustments to service plans as needed.
Advocate for clients' rights and needs within the community and among service providers.
Engage in community outreach to promote available services and gather feedback.
Collaborate with healthcare providers and community organizations to ensure holistic care for clients.
Requirements
Bachelor's degree in Social Work or a related field; Master's degree preferred.
Current state licensure or certification in social work.
Minimum of 3 years of experience in a social work role, with a focus on client coordination.
Strong understanding of community resources and social service systems.
Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
Proficiency in case management software and Microsoft Office Suite.
Demonstrated ability to work independently and as part of a multidisciplinary team.
