Summary
To lead the MDT ensuring high quality of therapy intervention and coordinated approach to maximise rehab potential. To negotiate with others around case management in complex cases To ensure the implementation highly specialist programmes of care and treatment to meet the patients needs. To adapt practice to meet individual patients circumstances, including due regard for cultural and linguistic differences To ensure the team demonstrate high level of clinical effectiveness by use of evidence based practice and outcome measures, ensuring that these are embedded in the practice of all clinicians in the service area (to include PROMS/PREMS) To provide professional support and advice to Specialist Neuro Therapies and to staff from other professional and organisational groups (e.g. nursing staff, social services staff) working with this care group. To contribute and facilitate the development of specialist care protocols/packages to improve patient care using QI where possible To contribute to working groups and policy development groups appropriate to service area and report findings and proposals to line manager To attend trust senior lead meetings To attend NEL meetings To ensure that the MDT staff area are undertaking necessary levels of CPD and reflection in order to maintain professional registration with the AHP within a framework of audit. To participate in workforce planning for the service in liaison with the service manager
