Summary
An exciting opportunity has arisen for a Senior Registered Nurse to join our Specialist Frailty Services in North East Essex, you would be part of multidisciplinary team providing specialist frailty input into our Emergency Department team at Colchester Hospital, and working in the Frailty Rapid Assessment Service and the Frailty Hospital at Home team (virtual ward) covering North East Essex.
We are looking for a talented Registered Nurse with commitment and enthusiasm who is willing to learn and has the ambition to be the best they can be in raising the profile of Frailty Services.
Proposed Interview date 14th April 2026
Toidentify, assess, and co-ordinate admission avoidance for frail patients
ensuring identified nursing needs are met across the frailty services.
To provide specialist advice and supervision to other professions outside
and within the multidisciplinary teams and across organisational
boundaries.
To act as a key worker and use assessments, pathways and processes to
provide a wide range of nursing, therapy, social and voluntary sector care
assessments for patients who may be at risk of hospital admission.
To ensure appropriate assessment, planning and implementation of care
and assist in the interpretation of information relevant to the patient’s
condition/progress identifying problems and needs of patients, and set
realistic goals.
Take responsibility, using a key worker approach, for integrating, co-
ordinating and proactively monitoring patients’ health and wellbeing
needs, so that the individual receives the best possible care in the most
appropriate place. Refer to step up facilities where appropriate.
To take proactive steps in identifying and co-ordinating complex care
packages according to patient/carer need, with special regard to those
with complex nursing care needs.
