Summary
The Practice Nurse will deliver high-quality, evidence-based nursing care to the practice population, working both autonomously and as part of a multidisciplinary team. The role includes supporting new-to-practice nurses and other healthcare professionals, helping to strengthen integration across primary care, community, secondary, and social care services. The post-holder will provide care across key clinical areas including chronic disease management, immunisations, cervical screening, infection prevention and control, health promotion, and public health screening programmes. They will also contribute to population health management, supporting initiatives that improve patient access, reduce health inequalities, and enhance health outcomes. As an integral member of the team, the Practice Nurse will promote clinical best practice, participate in training and professional development, and support the review and implementation of clinical policies and procedures. Appropriate clinical supervision and day-to-day support will be provided by the organisation.
