Job Introduction Lincolnshire County Council has an exciting opportunity for anAdults Home CareSupportCoordinator (Internally - HomecareSupportBroker)within our Adult Care Brokerage Team, to aid in delivering the right level of care to enable people to remain in their own home. This opportunity is available as a job-shareworking Monday to Wednesday 18.5 hours. Monday- 7.5 hours- 8:45 to 17:15 (1 hour lunch). Tuesday - 7.5 hours - 8:45 to 17:15 (1 hour lunch). Wednesday - 3.5 hours - 8:45 to 12:15. About the role By joining our Brokerage Team, you'll become part of a service that is central to supporting adults across Lincolnshire to remain independent in their own homes. You'll be at the heart of meaningful decision making every day, helping ensure care is delivered safely, quickly, and with dignity. You'll be arranging home care and support packages for adults across Lincolnshire. Think of yourself as the matchmaker between people and the care they need. Adult Care and Community Wellbeing represents adult social care services, public health and Community Protection & Commissioning within Lincolnshire County Council. Our primary goal is to support people in Lincolnshire to have a good life. We will enable people to have greater resilience, maximising independence and focusing on people's strengths, whilst safeguarding vulnerable adults. We have a co-produced Adult Social Care strategy, and have developed a resilient Target Operating Model (TOM) to ensure we are efficient, effective and outcome focused, with people at the heart of everything we do. What you'll do You will be supporting people in the community that reside within the east of the county, which includes, Market Rasen, Louth, Boston & Skegness. However, as a community support broker you could be covering other parts of the county, depending on demands. Arrange effective home care and support packages tailored to individual needs Work collaboratively with social care professionals to ensure safe, timely support. Support hospital discharge pathways to help people return home quickly and safely. Coordinate ongoing maintenance packages with Prime Providers following successful reablement. Use your initiative and geographical knowledge of Lincolnshire to source complex care packages in rural or hard tor each areas. Contribute to the Adult Care vision of delivering high performance, efficient and person centred services. About the team We are a supportive and friendly team of sixteen which includes Brokers, Senior Brokers and a Team Manager. We support one another, share knowledge and experience, and are always willing to lend a hand. Our close-knit team consists of animal lovers, runners, charity shop rummagers, knitters, walkers, gardeners, artists and sport fans. What can you add to the mix? About you Were looking for someone who is: Highly organised and able to work to critical deadlines. A strong communicator ,confident in liaising with providers and colleagues. Comfortable problem solving in fast paced and sometimes challenging situations. Creativity - Knowledgeable about Lincolnshire's communities and geography (or eager to learn!).
