About the Role:
As a Senior Social Worker, you will be a key professional in our front‑door Adult Social Care service, providing leadership while staying close to practice.

You will:

Lead, mentor and supervise Social Workers and Social Care Coordinators.
Carry out complex assessments, safeguarding work and mental capacity assessments.
Model best practice through joint visits and coaching.
Guide early intervention and short‑term support using our strengths‑based Conversations Count model.
Work with adults from all backgrounds to maximise independence and wellbeing.
Build strong partnerships across health, voluntary sector and community settings.
5 days pro rota per month for Continuous Professional Development (CPD)

This is a role where your experience shapes outcomes — and your voice helps shape the service.

The Hub includes Social Work and Occupational Therapy pathways and manages all new referrals for Adult Social Care for people aged 18 and over. It is a busy, friendly, and supportive team working with individuals, families, and carers from diverse backgrounds.

You will work with individuals on a short-term basis, to help maximise independence at home, focusing on strengths, choice, and control.

This role involves supporting adults with a wide range of needs, including learning disabilities, physical disabilities, older age, and issues related to substance or alcohol misuse.

This position offers an excellent opportunity to develop your skills and experience in Adult Social Care, delivering high-quality, person-centred interventions to individuals in their own homes.

You will need to have:

A degree or equivalent in Social Work
Registration with Social work England
Strong practice knowledge of the Care Act 2014 and MCA 2005
Experience in safeguarding adults
Confidence, curiosity and a passion for strengths‑based working
