Summary


The role of Outreach Wellbeing Coach (OWC) offers an exciting opportunity to work for Provide on behalf of the Essex Wellbeing Service.

As an Outreach Wellbeing Coach, you will work in partnership with other providers to improve the health and wellbeing within seldom heard from communities across Essex. Your focus will be on delivering one-to-one coaching, health promotion sessions and health checks, support with smoking cessation, weight management and the general well-being of your clients.

Due to the nature of the role, there will be a need to work some evenings and weekends to deliver outreach events such as Prides and other festival type events.

The role is an in-person face to face role which requires peripatetic working with frequent travel. The applicant will need to hold a full UK drivers’ licence and have access to a vehicle.



Experience of community work is essential to this role, and we welcome and encourage job applications from people of all backgrounds.

The successful applicant will need to evidence relevant skills and experience required to deliver this role successfully, however at the start of employment we will provide comprehensive training, including a Health Coaching qualification, plus NHS training to deliver Health Checks.

EWS is delivered by an alliance of specialist organisations to Essex residents who opt to receive health and wellbeing support. Whether it's help with social isolation support or help with day-to-day needs; EWS is here to help all Essex residents stay healthy, safe and well.



OWC work across EWS to reach individuals from seldom heard from communities.



We offer and deliver wellbeing coaching, health checks, smoking cessation and other interventions. Our ultimate aim is to improve our clients’ health and wellbeing and increase their connections within the community for long term benefits and cross-community positive impact.



OWC specialise in working with the following client groups: those currently homeless or in unstable accommodation. Those coming through the Criminal justice system. Traveller communities. Racially Minoritised Groups. Those with a history of drugs and alcohol use. LGBTQ+ communities. And those who have lost trust in services and have opted to disengage.



Although the successful applicant will have to evidence the relevant skills and experience required to deliver this role successfully, at the start of employment we will provide comprehensive training, including an accredited Health Coaching qualification, plus NHS training to deliver Health Checks.

● To work closely with target populations to gain trust and engagement and to find people who will benefit from support but are not likely to access the service via the normal referral routes.



● To support the development and delivery of health promotion (HP) and behaviour change communication (BCC) interventions in Essex targeting hard to engage populations.



● To deliver a tailored 12:1 coaching programme for hard to engage groups that will improve individual’s health and wellbeing and increase their connections within the community for long term impact.



● To support with self-management to work on client’s barriers, improve their self-worth and confidence. Support with smoking cessation, accessing health checks and weight management



● To explore opportunities for individuals taking part in the coaching programme to engage with volunteering options in the local community, as part of the EWS strengthening communities work plan.



● To be a continuous point of contact for as long as needed and then in addition provide follow ups at set intervals for as long as necessary to sustain outcomes. Provide support with connections within their community.



● To facilitate outreach HP/BCC interventions (face to face or virtually) with target populations and partner agencies. This includes developing / delivering training sessions for practitioners and group sessions for service users as required.



● To develop, maintain and use systems to monitor and record HP/BCC interventions and contribute data for reporting as requested. Adapt interventions in support of service KPIs and contractual obligations. Use the EWS Priority Platform for all data recording.



● Manage a caseload of individuals determining the contact points, frequency and end point. Attend multidisciplinary team (MDT) case load discussions if required.



● To establish and maintain professional relationships with partner agencies and to professionally represent EWS and Provide with target populations and partner agencies during HP/BCC interventions, adhering to Provide values and policies.



● To identify and undertake continuing professional development (CPD) to improve skills and maintain a good working knowledge of health and wellbeing aspects to meet the needs of target populations and partner agencies.



● To use own initiative to work independently, to achieve personal work objectives linked to the annual delivery plan. Attend team meetings and support work requests from the EWS locality lead.



● To manage own workload, maintaining and updating calendars, timesheets, annual leave records, and to notify the line manager of any change in personal circumstances or personal information relevant to the role.



To manage work social media accounts to promote EWS, local events and initiatives, linking in with other appropriate organisations
