Summary
Duties and responsibilities Be skilled in Aseptic Non-Touch Technique and be able to provide after-care for the lines across the trust, placed by the vascular access clinical nurse practitioners or by the interventional radiology team. Recognise the need for escalation when managing the deteriorating patient in the Units, ward or departments. Ensure the environment is clean and prepared for, including stock levels for the treatment trolley Deliver effective nursing care to patients receiving PICC line insertion, midline insertion and post-insertion care under the direction of the Vascular Access CNS. Communicate effectively with patients and all members of the clinical teams to ensure information is shared in a timely and appropriate manner where appropriate Participate in the Trust Appraisal process to ensure that you receive and give constructive feedback to enhance personal skill set Take all reasonable precautions to ensure that you do not endanger yourself or others by act or omission Assist Registered Nurses and clerical staff in administrative duties, such as conveying information from telephone or personal enquiries, maintaining a courteous manner at all times At all times, contribute to the maintenance of the health, safety and security of individuals and their environment, reporting to the nurse in charge any hazards which may endanger individuals and where suitable take appropriate action At all times, adhere to policies and procedures of the Trust Demonstrate a basic understanding of the reasons for blood tests and knowledge of the results and the importance of timely escalation of results. Carry out blood collection from fixed or central lines in accordance with competency and trust policies and procedures Be competent to access the equipment for all forms of vascular access devices such as PICC lines, midline catheters, port-a-cath and Groshong lines. Be competent to change PICC dressings in accordance to Trust Policy Safely care for individuals with long term IV access devices (e.g. PICC/porta-cath) Demonstrate an understanding and your role in the recognition of neutropenic sepsis COMMUNICATIONS & RELATIONSHIPS Establish and maintain effective communication with patients, significant others and patient groups. Develop and maintain appropriate communication with members of the multi-disciplinary in all aspects of care delivered. Demonstrate a variety of communication skills with patients who may have communication difficulties e.g. confusion, dementia or other linguistic issues Demonstrate good communication skills in difficult circumstances. Ability to deal with and respond appropriately to concerns or complaints from patients or members of the public and know who to escalate the concerns to. Maintain clear, concise and legible documentation adhering to standards in accordance with trust policies and guidance. Ensure compliance with trust policies and procedures regarding confidentiality of data and data security. Communicate with colleagues and members of the multi-disciplinary team clearly and effectively in written, verbal and electronic format. PHYSICAL DEMANDS OF THE JOB Frequent standing, sitting, walking kneels and crouches to attend to patient needs. Responding to changes patient condition. Moving & Handling of patients and equipment on a frequent daily basis. Frequent mental effort/concentration required in relation to responding to and meeting patients nursing care needs (e.g. for 7.5hr hour shift) able to follow routine and/or direction by registered practitioner. Verbal and written communication skills required. Exposure to difficult situations including unwell/ distressed patients and relatives/carers This job description may be subject to change according to the varying needs of the service.
