Summary
We are looking to appoint a Locum Consultant Physician in Nephrology & General Medicine. The ideal candidate should have an interest in all aspects of nephrology care. They will encompass a combined renal service with Walsall Together and there will be a requisite to partake in acute medical cover, renal inpatients cover and the teaching and supervision of junior medical staff.
We are looking for a candidate who can work collaboratively with GPs, Palliative Care and Royal Wolverhampton NHS Trust Hospital renal service.
The post is for 10.25 Programmed Activities per week which include participation in GIM on call rota.
Walsall Healthcare NHS Trust renal service is linked to Royal Wolverhampton NHS Trust Hospital, Wolverhampton. There are 7 outpatient nephrology clinics at the Manor Hospital with medical input from visiting Nephrologists and renal specialist nurses from Royal Wolverhampton NHS Trust who are in attendance at 2 clinics each week.
Inpatient Renal service is provided by our Consultant Nephrologists Dr Riaz Bavakunji and Dr Thahir Abdul Samad and Dr Huda Mahmoud. There is a satellite haemodialysis unit in Walsall run by Diaverum, Green Lane, and Walsall, which is not a requirement of the post holder to support.
Daily Ward Rounds on Ward 15 (according to Rota). This ward is currently shared between two consultants at any one time during hot week. General medical, nephrology, diabetes and endocrine patients are admitted to these 28 bedded wards. Delivery of outpatient clinics (3 per week).
2. Inpatient Acute Kidney Injury service for all stages 2 and 3 AKIs with help of automatic AKI stage reporting software in order to improve outcomes which was implemented in May 2015. There are three full time AKI nurses for daily reviews, providing a 7-day service.
3. To assist in the Cardio renal service in the Trust in collaboration with the Cardiology department.
4. To participate in primary care education about chronic kidney disease and develop shared care strategies with primary care. To be involved in database work and reviewing all CKD with eGFR < 30 within the hospital who are admitted with other medical/surgical issues and advise as necessary. This is part of initiative to reduce late referrals and to aid GPs monitoring and managing patients in the community.
5. To be part of Education programmes for Renal Medicine for undergraduate students and junior doctors. Be active in the junior doctor's induction and keeping them up to date with AKI and other relevant guidelines. Interest in MRCP teaching is desirable.
Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 284,300, providing inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community. Walsall Manor houses the full range of district general hospital services under one roof. The £170 million development was completed in 2010 and the continued upgrading of existing areas ensures the Trust now has a state-of-the-art Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit facilities.
A new Urgent Emergency Care Centre was opened in March 2023. The two-storey development has significantly improved emergency care facilities and capacity and has provided almost 5,000 square metres of additional clinical space.
This is the main responsibility of the post holder. Acute Kidney Injury requiring emergency renal replacement therapy is jointly managed by the ITU team at Walsall, Walsall Nephrologists and Wolverhampton depending on stability for transfer. If stable, patients are transferred for ongoing renal replacement therapy for single organ failure to Royal Wolverhampton NHS Trust Hospital. There is a strict transfer policy in place to ensure patient safety.
Walsall currently has 3 AKI nurse who are proactively providing input to AMU and surgical wards and who provide a 7-day renal cover service. The post holder will work with the AKI Team to deliver this service and facilitate early discharges to follow up in the AKI hot clinic.
