Summary
1.Interdisciplinary working with other health professionals for holistic assessments and treatment plans including assessment, goal setting, planning and developing therapy treatment programmes; monitor and review, as part of the Physiotherapy component of the D2A Pathway 1 Service. Responsibilities of this post holder will include the following: 1.To write and review treatment plans to enable patients to maintain maximum independence. 2. To undertake treatment plans identified by physiotherapists. 3.Act at all times in a manner which respects privacy, dignity and confidentiality. 4. To maintain patients notes in accordance with Trust policy. 5. To manage own caseload, time management and diary arrangements. To manage, supervise and coordinate more junior physiotherapy staff. Planning and delegating their workload as appropriate. Provide guidance and delegated tasks to support workers conducive to the individual rehabilitation programme delivered within the Home First Team Triage referrals to determine appropriate level of physiotherapy input required to meet individual patient needs. Support those who require rehabilitation assessment and treatment, to prevent hospital admission and maximise functional independence. Support those who have been referred from hospital to discharge to assess pathway 1 for assessment and rehabilitation within their own home or in reaching in order to avoid an acute admission into Community Hospital bed. To advise and communicate effectively with patients (who frequently have problems with understanding), carers and other members of the multidisciplinary team, to maximise rehabilitation potential. Monitor the individuals progress and amend the aims and goals of treatment accordingly. Communicate effective complex information to members of the Multidisciplinary Team.
