Summary
Lead Diabetes Role Within Meanwood Group Practice, the post holder will: Lead and coordinate diabetes care pathways Manage Type 2 and pre-diabetes Undertake comprehensive annual diabetes reviews Initiate and titrate insulin and injectable therapies (if qualified) Interpret HbA1c, lipid, renal and ACR results Perform diabetic foot assessments Develop personalised care plans Reduce health inequalities within the practice diabetes population Educate and train other team member Be proactive in implementing service improvements for diabetes Practice Nurse Responsibilities The post holder will deliver comprehensive general practice nursing services including: Chronic disease management (Asthma, COPD, Hypertension, CKD, CVD) Cervical cytology Childhood and adult immunisations Wound care and leg ulcer management Contraception and sexual health services NHS Health Checks Health promotion and lifestyle support Infection prevention and control compliance Travel Vaccinations
