Summary
The ANP will have the following key job responsibilities, in delivering health services; 1.They will work as part of a multi-disciplinary team within the practice. 2.Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis. 3. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly 4. They will advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the practices social prescribing service, and where appropriate, other community or voluntary services. 5. Provide clinical leadership within the practice 6.Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice 7.Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. 8. They will communicate at all levels across organisations ensuring that an effective, person-centred service is delivered. 9. On occasion they will provide same day home visit to housebound patients. 10. They will communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required. 11. They will maintain accurate health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice. 12. Contribute to quality improvements 13. Communicate effectively with patients, and where appropriate family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings, and treatment choices. 14.Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan. 15.Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment 16. Communicate with and support patients who are receiving bad news 17. Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating 18. Anticipate barriers to communication and take action to improve communication 19. Maintain effective communication within the practice environment and with external stakeholders 20. Recognise and work within own competence and professional code of conduct as regulated by the NMC. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures 21. Deliver care according to NICE guidelines and evidence-based care 22. Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation 23. Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities 24. Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required 25. In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate 26. Support and participate in shared learning across the practice and wider organisation 27. Use a structured framework to manage, review and identify learning from patient complaints, clinical incidents and near-miss events 28. Assess the impact of policy implementation on care delivery 29. Understand and apply legal issues that support the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patients health procedures and local guidance Safeguarding Children and Adults The Chislehurst Partnership is committed to safeguarding and promoting the welfare of children, young people (anyone under the age of 18) and adults.
