Summary
Demonstrate and further develop specialist knowledge in Glomerulonephritis (GN), Vasculitis, and Polycystic Kidney Disease, including the interpretation and understanding of relevant blood test results. Work collaboratively with the multidisciplinary Renal team, including Nephrologists, to enhance specialist knowledge, skills and patient outcomes Support patients in the management of kidney conditions with the aim of relieving and controlling symptoms and reducing disease progression. Develop and apply advanced assessment skills to identify patient needs, providing tailored education, information, advice and counselling. Provide cross-cover for the Remote Monitoring Service as required Liaise with other Renal services to benchmark practice and share learning to promote continuous improvement. Support service delivery across the Berkshire Kidney Unit, including attendance at clinic across East and West Berkshire Royal Berkshire Hospital, West Berkshire Hospital, Bracknell Health Space, Townlands Hospital and Windsor Dialysis Unit. Provide expert nursing care and specialist advice for patients diagnosed with Vasculitis, Glomerulonephritis (GN) and Autosomal Polycystic Kidney Disease
