Summary


The post holder will be required to work under the guidance of the Unit Manager undertaking tasks required in respect of discharge planning, general administration and housekeeping within the department.

Whilst working under the guidance of the Unit Manager the post holder will be required to organise their own workload and use their own initiative to ensure required tasks are completed.
The primary function of the role will be assisting the nursing and therapy teams in the promotion of flow into and out of Elmhurst to expedite discharges, taking handovers for patients referred for triage by the Nursing team and using judgement to determine appropriate nature of patient referral following discussion with the nurse in charge, liaising with transport, care agencies, providers and families.
Additionally, the post holder will also need to provide a comprehensive administrative service to the department and undertake housekeeping duties as required and taking into account the needs of the patients within the department. This would include being responsible for the co-ordination of all patient non-clinical facilities/services. Ensuring a clean, safe and comfortable environment within the Elmhurst by ensuring quality standards are continuously monitored and estates issues are
promptly rectified. Effective stock monitoring, ordering and management.

SERVICE DELIVERY
1. Take a supporting role in assisting the nursing team to ensure an element of the systems and processes of admission and discharge planning are expedited efficiently and effectively to maintain patient flow.
2. To identify issues that may delay a discharge that require escalating to the nurse in charge/doctor/matron etc.
3. Promote and maintain a safe and therapeutic environment for patients, respecting their views and beliefs, whilst awaiting discharge home/admission to the ward.
4. Establish and maintain excellent communication, informing patient and significant others of time delays and organisation of admission and discharge.
5. Discuss and support the nursing team with plans for discharge with the co-operation of patients and their families/carers.
6. Ensure the legibility and accuracy of referrals to all health professionals.

Job Description - Some of the Key elements of the Job Role:
1. Provide an efficient reception service, greeting patients, public and all visitors as they arrive, dealing with enquiries promptly, taking action as necessary and directing patients/relatives/carers accordingly.
2. Provide a comprehensive clerical service to the Divisions, undertaking all relevant work-related tasks including retrieval, preparation and filing of patient information/data, correspondence, photocopying, faxing and emailing, distributing reports and letters according to the requirements of the service, where appropriate.
3. Maintain systems and process to ensure that patient information/data is available at the right time, that they are in the right place, and that they are complete and accurate according to the agreed quality standard.
4. Update patient information/data when patients have been seen, according to the agreed quality standard, locate and retrieve supplementary documentation for patient information/data as required, filing diagnostic reports and essential documents in the appropriate place.
5. Be able to use the required electronic systems for correspondence, clinic management, as and when required.
6. Maintain constructive relationships with others, within and across Divisions and externally in order to improve the effectiveness and efficiency of services to patients.
7. Other Ad-hoc duties relevant to the job role and band.
