Summary
Location: Watton, Norfolk
Reports To: Home Manager
Role Overview:
As a Care Coordinator, you will play a vital role in delivering high-quality, person-centred care to our residents. You will work closely with the care team to ensure the coordination and management of care plans tailored to each resident's needs, preferences, and goals. Your responsibilities will include shift organisation, medication administration and recording, liaising with healthcare professionals, and maintaining up-to-date care plans. You will also assist with the induction, training, and supervision of support staff.
Key Responsibilities:
- Coordinate and manage care delivery to meet residents’ individual needs.
- Organise and manage shifts to ensure adequate staffing and smooth operations.
- Administer and record medication accurately, in compliance with guidelines.
- Liaise with healthcare professionals to ensure continuity of care.
- Maintain accurate, up-to-date residents' care plans in the computerised system.
- Support the induction, training, and supervision of care staff to maintain high service standards.
Skills and Attributes:
- Experience in supervising a care team and providing leadership.
- Empathy and a deep understanding of residents’ needs.
- Strong communication and organisational skills, with attention to detail.
- Ability to work collaboratively as part of a team.
- Flexibility around work schedules and responsibilities.
Education and Qualifications:
- NVQ3 in Health & Social Care (or working towards).
