Summary
Summary Leading across the practice in effective medicines management and best practice prescribing, as well as audit and quality improvement work, our Pharmacy Lead shares pharmacy expertise to support GPs and the whole practice to effect optimal patient care. This role is integral to the practice, leading and mentoring the pharmacy team and progressing and developing the pharmacy function throughout the practice. Core duties and responsibilities Medicines reconciliation and repeat prescription management, including implementing changes in medicines in line with local and national guidance or shortages Undertaking medication reviews, including Structured Medication Reviews with patients both face to face and telephone with patients and remotely ensuring best practice care management for patients with chronic diseases proactively managing complex polypharmacy, especially in the elderly, people in care homes, those with multiple co-morbidities people with learning disabilities Clinically assess and treat patients using expert knowledge of medicines for specific disease areas Supporting the management of chronic disease - both remote and patient facing Inform and advise colleagues as relevant about ongoing prescribing issues, new guidelines (national and local), new products, secondary care protocols, manufacturing and supply problems, new prescribing restrictions, or contraindications Take a central role advising and supporting prescribers in following local formulary and shared care protocols, promoting cost effective prescribing Support the prescribing and management of medicines in our managed care home Support and, where appropriate lead, on QOF, enhanced service delivery, IIF and local prescribing quality scheme Identify, develop and implement change across the practice in relation to medication management, safety, monitoring and prescribing Lead Help in development and maintain maintenance of internal policies and processes relating to medicines, medication monitoring and prescribing Liaison with community pharmacies Work with and support the Dispensary Lead as appropriate Manage, mentor and support the education and training of the clinical pharmacy team Lead on progression and development of clinical pharmacy service within the practice Other duties and responsibilities Effectively lead, manage and evaluate change Demonstrate and implement knowledge of National and Local Health policy issues and their implications to the service Develop and review existing evidence based clinical protocols/guidelines that drive clinician led interventions Critically analyse data/information to inform/change patient care practices Develop and maintain appropriate documentation/records in accordance with practice policy and procedures Maintain own professional and personal development including revalidation requirements Assist in the recruitment and training of new members of both the Pharmacy and Dispensary team Research and report writing as required Participate in training and CPD as necessary Ad hoc assistance to the Practice Manager and Partners as appropriate Any other delegated duties appropriate to the post Person specification Flexible with a positive, can-do attitude and comfortable juggling a varied workload A natural problem solver able to work well under pressure Practical, unflappable and confident Attention to detail Strong organisational skills Excellent written and verbal communication, able to edit and disseminate information for different audiences
