Summary


To assist all members of the multi-disciplinary staff to provide a high standard of patient center care including assisting patients to wash and dress, toileting, eating and drinking. All activities of daily living.To assist patients to achieve independence and their maximum independence through a planned programme of rehabilitation and re-enablement.To contribute to the effective and efficient management of all unit resources.To provide a safe and supportive environment in which patient centered rehabilitation can develop


Tofacilitate and support patients communication throughout daily activitiesAssist and run individual or group activities as requiredAssist patients to gain adequate nutrition by serving meals/drinks taking into consideration dietary/nutritional requirements and helping with eating and drinking as neededTo assist patients carrying out exercise programs relating to mobility, communication, feeding and drinking as directed by members of the multi-disciplinary teamAssist patient in transfer practice and mobilizationTo assist members of the multi-disciplinary team in discharge planning and assessments for equipment where appropriateTo take telephone messages accurately and relay messages to appropriate personnelTo comply with health and safety policies, reporting accidents/incidents, ‘near misses’ or hazards in accordance with agreed local procedures and CHS policies


Windsor intermediate care unit is a 19 bedded unit. Our purpose is to provide an enhanced intermediate care and rehabilitation service predominately for the older person. Preventing acute hospital admissions and facilitating early discharge are our primary goals. We are currently looking for a motivated, confident and forward thinking individual to join our well established care and therapy team.

Assist patients in daily activities, including washing, dressing and kitchen skills as directed by members of the multi-disciplinary teamAssist patient with elimination needs and personal hygiene maintaining their privacy and dignityTo prevent/minimise cross infection by adhering to infection control policiesMinimise the risk of pressure ulcers developing through own practice and assisting with the education of the patientTaking and recording temperature, pulse, respiration rate, blood pressure and blood sugar readings, as directed by a Registered nurseTo collect relevant samples/specimens as requested by registered nurseMaintaining effective communication networks within the team/unit, ensuring confidentiality at all timesDeveloping and maintaining skills in basic life supportTo ensure effective use, care and maintenance of equipment within the unitTo assist with the serving of meals and drinks provision, ensuring that all patients receive adequate assistance/support to achieve nutritional needsTo participate in bed making/cleaning as required
