Summary

Job Description Job Title: TDDI Nurse Location: Watford, Hertfordshire (covering surrounding areas) Reporting to: Registered Manager Work Pattern: Part Time / Flexible Job Purpose To provide clinical oversight and nursing input for individuals receiving Treatment of Disease, Disorder or Injury (TDDI) within domiciliary care and supported living settings. The post holder will support safe delivery of complex care in community environments, ensuring appropriate clinical standards, governance and accountability are maintained. The role combines clinical input, oversight and leadership to support safe outcomes for individuals with complex and long term health needs living outside hospital settings. Key Responsibilities Clinical Oversight Provide nursing input and clinical oversight across TDDI care packages within domiciliary and supported living services. Support safe implementation of nursing interventions delivered in community environments. Monitor individuals health status and contribute to escalation where deterioration or clinical concern is identified. Care Planning and Clinical Support Support development and review of clinically informed care plans. Ensure care delivery reflects current clinical needs and supports safe outcomes. Provide guidance to care teams to ensure care is delivered in line with agreed clinical standards. Medication and Health Management Support safe medication practices within TDDI frameworks. Provide clinical advice relating to health needs, treatment and ongoing monitoring where required. Workforce Support Provide clinical guidance to non registered staff. Support competency development where clinical tasks are delegated. Promote safe working practices within care delivery teams. Quality and Governance Support clinical audits and quality assurance activity. Contribute to risk management processes. Maintain appropriate records in line with professional standards. Work collaboratively with the Registered Manager to support safe service delivery. Partnership Working Liaise with GPs, community services and other health professionals where required. Support coordinated care across health and social care settings. Professional Responsibilities Maintain active NMC registration. Work in line with the NMC Code and professional standards of practice. Maintain confidentiality and adhere to data protection requirements. Act within scope of competence at all times. Person Requirements Registered Nurse with current NMC registration. Experience supporting individuals with complex health needs. Confidence in working within community environments. Ability to support and guide care teams. Strong communication and organisational skills. Full UK driving licence with access to a vehicle.
