Summary
What we're looking for Experience in chronic disease management (e.g. Diabetes, Asthma, COPD, Hypertension, CVD) is an advantage, but training will be given to support your development into LTC nursing Confidence working independently as well as within a wider multidisciplinary team Strong clinical judgement and excellent organisational skills A proactive, flexible approach, especially during busy periods Ability to handle sensitive or confidential patient information professionally Ideally a nurse prescriber with primary care experience (desirable but not essential) We welcome both full or part-time applicants, experienced LTC clinicians, and those who are looking to develop their career in long term condition management.
