Summary


Are you a forward thinking Staff Nurse looking to move into the exciting area of specialist neuro-rehabilitation? If so, an opportunity has arisen within the Specialist Rehabilitation Team based at the Colman Centre for Specialist Rehabilitation in Norwich.

Our team provides specialist neuro-rehabilitation for patients who are referred to us from across the East of England. We are looking to appoint an enthusiastic, well motivated staff nurse, into this full time post. You will cover a combination of earlies, lates and nights. Long days are also available. You will work as an integral part of the ward team and be responsible for providing high standards of rehabilitation care to a group of patients with specialist needs

You will be required to deliver evidence based care to a high standard. Excellent communication skills and a supportive approach to team working within a rehabilitation setting are key to the success of this service.

An ability to structure your own learning supported by the team gaining new knowledge and skills to benefit yourself and the service will be vital. Some of the skills you can develop may include enteral feeding, tracheostomy management and weaning, and posture and position management. In addition you will be working with families and colleagues in complex discharge planning and best interest discussions.

Excellent organisational and interpersonal skills are essential as is the need to be cheerful and courteous. Good computer skills or the ability to learn will be required for this role as we are using SystmOne to record our patient interventions

With effect from the 1stApril 2026 Norfolk Community Health & Care NHS Trust will be merging with Cambridgeshire Community Services NHS Trust to form a new organisation. The new Trust will provide a wide range of community health and social care services to over 4 million people, with a budget of circa £380 million and a workforce of over 7,000 people.
• To assess, plan, implement and evaluate care of assigned patients.
• To initiate this process when a patient is planned for admission to the unit ensuring the correct information, equipment etc. is in place and commences the assessment process.
• To provide care for patients as defined by the individualised care plan, reviewing and updating this when care needs change.
• When necessary, to make changes to planned care when the need arises and change the care plan accordingly. To communicate the changes to the wider team to ensure the correct care is delivered.
• To accurately document all stages of the nursing process.
• To keep care plans and patient records up to date, reviewing these as care changes.
• To provide a high standard of nursing care to the patients and evidence this.
• To establish a relationship in which the patient, family and friends may express their needs, and have those needs met where nursing can provide.
• To involve the patient, family and friends in setting goals, evaluating and taking part in the care process.
• To act as patient advocate where appropriate, ensuring patients capacity is assessed working with the mental capacity Act.
• To support, offer help and be aware of other staff members needs, and ensuring they deliver care appropriately based on the needs of patients as identified in their care plans.
